Four Levels of Care
Every client is assigned a nurse case manager from day one — who conducts an initial assessment, creates a customized care plan, and remains actively involved throughout. Aides are carefully matched to each client, both clinically and personally. Scheduling is flexible, from a few hours a day to 24/7 care.
Companion Care
Transportation to appointments, help with household tasks, grocery shopping, laundry, errands, and the consistent human presence that makes living independently sustainable and enjoyable.
Home Health Aides
Everything companion care offers, plus personal in-home care — assistance with meals, bathing, dressing, and other activities of daily living. Matched carefully to each client, clinically and personally.
Registered Nurses & LPNs
Health assessments, safety evaluations, wound care, and comprehensive medication management — including portioning, education, and physician coordination. An RN supervises all in-home care throughout.
Heartfelt Connections™
A specialized memory care program in which all GlenCare clinicians and caregivers are trained — focused on enriching the lives of those living with memory challenges with dignity and purpose every day.
